About the Program

The COACH Program leads the world with its evidence-based, award winning coaching prevention program for people with chronic disease or at high risk of chronic disease.

The COACH Program is a standardised structured coaching program delivered by highly trained health professionals (“coaches”) to people already diagnosed with a chronic disease or who are at high risk of developing chronic disease.

The COACH Program is delivered by telephone and mail-outs over a period of 6 months. The Program is focused on closing the “evidence-practice gap” – the gap between guideline-recommended care and the care patients actually receive.

Developed in Australia in 1995, The COACH Program has been rated the most evidence-based cardiovascular disease prevention program in the world on clinical and cost-effectiveness, by the British Heart Foundation and Public Health England’s “International Cardiovascular Disease Prevention case studies report – October 2018”.

The COACH Program was selected from a field of 118 programs across the globe.

Supported by the highest level of evidence with published randomised controlled trials and several long term follow-up studies, The COACH Program has been proven to help people with chronic disease live longer.

What are ‘risk factors’ to health?

A risk factor is anything that increases your chance of getting a disease. For instance, high cholesterol increases your chance or ‘risk’ of getting cardiovascular disease (including heart attack and stroke) – this means high cholesterol is a ‘risk factor’. Other known risk factors for cardiovascular disease include smoking, high blood pressure, diabetes, overweight, poor diet and nutrition, physical inactivity and excessive alcohol consumption.

Improving risk factors retards the disease process and prolongs life

For people with chronic disease or at high risk of chronic disease, the successful improvement of disease risk factors and adherence to recommended medications are the only proven effective treatments that are known to retard the disease process and prolong life.
