Achmea, the largest private health fund in The Netherlands has launched The COACH Program which will be made available to more than 5000 patients per year with coronary heart disease.
Queensland Health has introduced The COACH Program through their Health Contact Centre to provide coaching to people recently discharged from hospital following a cardiac event or cardiac procedure.
There is a ‘gap’ between the guideline recommended treatments patients with chronic disease should be receiving as against what they are actually receiving.
